【Application Procedure】The individual applicant or contact person of a group will receive a payment email within 3 working days of the application. Please use FPS to settle the payment within 3 working days. (If you are unable to pay via FPS, please contact us at 2705-2397 or email us to request an alternative payment method.)

【Remarks】
1. No lunch break will be arranged during the activity. Participants are advised to have their meal before the programme begins.
2. The activity will be conducted in teams of six participants. Individual applicants will be assigned to a team by the organiser. Applicants registering in groups of two to five may indicate their accompanying participants in the application form. The organiser will make every effort to accommodate such requests but reserves the right to make final adjustments to team arrangements.
3. This is an experiential learning activity that requires a certain level of physical fitness and includes simulated conflict scenarios and role-playing elements. The programme aims to enhance participants' understanding of and reflection on humanitarian issues. As some content may involve sensitive or emotionally challenging topics, certain participants may experience emotional discomfort or distress. Participants are advised to carefully consider their physical and emotional well-being before registering and determine whether the activity is suitable for them.
4. The organiser will make every reasonable effort to provide a safe and appropriate environment for participants. Participants are also responsible for taking care of their own safety and personal belongings throughout the activity. Participants assume all risks associated with their participation. The organiser shall not be liable for any injury, accident, or loss of personal property arising from personal circumstances or events beyond the organiser's reasonable control.
5. Participants are required to complete a Health Declaration Form prior to the activity. If the organiser or its partner organisation considers a participant's physical condition unsuitable for participation, the organiser reserves the right to refuse the participant's continued participation in the activity. Any fees paid will not be refunded.
6. All programme fees are non-refundable once payment has been made, except in the event that the organiser cancels the activity. Membership fees are non-refundable under all circumstances.

IMPORTANT REMARKS

[Part A]

1) Once payment has been made, the programme fee will not be refunded except in the event that the Organiser cancels the programme. Membership fees are non-refundable under all circumstances;
2) For Group application, please assign one member to be the contact person of your group to fill in this form and settle all the payment. Please also reserve sufficient time (about 5-10 mins) for filling in this form;
3) The Organiser will arrange teams according to the actual registration situation and reserves the right of final decision;
4) Participants must attend and complete the entire event on time with satisfactory performance to be eligible for refund of the deposit;
5) For any discrepancy between the English and Chinese remarks, the Chinese version shall prevail.

[Part B]

Applicants must be the members of University & College YMCA. Please ensure that all applicants have completed membership registration and have both 1) the membership number and 2) the expiry date.

(ii) If you are not our member yet, please click HERE to know more and apply for our membership by:
1. [Credit Card] Access the online membership application portal and select "Uni-Y/ College Y Member" for membership type. Fill in and pay the relevant membership fee. You will instantly receive the membership number and expiry date via the confirmation email sent to your mailbox.
2. [FPS] Download and fill in the membership form for your institution, email it to uniyhku@ymca.org.hk with the subject named: "Membership Application - The War Journals". You will receive a payment instruction via email within 3 working days to settle both the membership and programme fee in one go.

*Each participant is required to submit a HKD$100 deposit. It will be fully refunded upon completion of the program with satisfactory performance.

^This activity will be conducted in groups of 6. Individual applicants will be paired up by the Organiser. Groups of 2 to 5 participants could indicate their co-applicants on the registration form; the Organisers will make every effort to accommodate group placements but reserve the right to make final team adjustments.

Important Notes and Declaration for 6-Person Teams

If any member of a pre-formed 6-person team is absent on the event day, the Organiser reserves the right, for safety reasons and to ensure the smooth running of the event, to assign individual participants to join your team. The Group Contact Person is responsible for ensuring that all accompanying participants have been informed of and understand this arrangement.

Terms and Conditions

1. Activities involve physical exercise. All participants must ensure that their physical and mental conditions are ready for this activity. The Organiser is not liable for any injuries/ sickness/ loss of personal belongings during the event; participants have the sole responsibility instead; 活動涉及體能活動，參加者需確保身體狀況良好並適合參與是次活動，主辦單位並不會對參加者所發生的任何意外、受傷或其所攜帶之財物承擔任何責任。參加者須自行承擔一切責任；

2. Participants under 18 years old on the Event Day must submit a consent form signed by their parent/guardian; 於活動當日未滿十八歲之參加者必須於活動當日或之前遞交由家長或監護人簽署的同意書方可參與活動；

3. Photos and videos will be taken at the event for future promotion and the Organiser reserves all copyright and ownership of the photos and videos taken at the event; 主辦單位於活動期間所拍攝的相片或影像，將用作日後宣傳及推廣用途，並保留有關相片及影像的版權及擁有權；

4. Limited quota applies. Your application will only be confirmed upon the completion of the payment. The Organiser reserves the right to close entries before the announced deadline without prior notice; 活動名額有限，一切以繳費為準，主辦單位有權提前截止報名而不作另行通知；

5. The Organiser reserves the right to contact participants by phone or other means for additional information required for the application; 主辦單位有權以電話或其他方式聯絡參加者以取得所需資訊作活動安排用途；

6. The Organiser reserves the right to void any applications with invalid information and/or submitted via improper procedures; 若參加者提供的資料有任何錯誤或不依照正確程序報名，主辦單位保留拒絕接受其參與權利；

7. The Organiser reserves the right to cancel the activity due to any circumstances beyond the organiser's control, including but not limited to insufficient enrolment, severe weather condition, social unrest, severe pandemic etc. while all application fee will not be refunded; 如發生任何主辦單位無法控制的情況導致活動無法如常舉行，包括但不限於參與人數不足、惡劣天氣或特殊社會及疫情狀況等，主辦單位有權決定取消整個活動，所遞交之報名費不獲退還；

8. The event will be cancelled if a red or black rainstorm signal or a tropical cyclone signal No.3 or above is still hoisted on or before 1:00 p.m. on the event day. The cancellation announcement will be posted on the Facebook page and Instagram page of University & College YMCA. Participants are fully obliged to look for such announcements on their own; individual notifications will not be made. If an alternate mode of the Programme is adopted due to any conditions (including but not limited to the adverse weather conditions mentioned above , severe pandemic situation, etc.), the Organiser will rearrange and send the notifications to all participants; 如活動當日中午9時或之前天文台仍懸掛三號或以上熱帶氣旋警告信號、紅色或黑色暴雨警告訊號仍然生效，當天活動將會取消。相關通告將於大學及學院青年會 Facebook 專頁和Instagram賬戶發佈，參加者有責任自行到專頁瀏覽。主辦單位將不會個別通知參加者。如活動受任何情況（包括但不限於以上惡劣天氣或疫情等影響）而需改為以其他形式進行，主辦單位將會另作安排及通知各參加者；

9. Participants must follow all instructions given by the event staff, volunteers and officials with respect to all matters regarding the event; 參加者必須尊重及遵守由主辦單位職員、工作人員或其授權人士所發出的任何指引；

10. The Organiser reserves the right to prohibit the participants from joining without any refund of application fee if the participants violate any of the above conditions and terms; 參加者如違反上述條款，主辦單位保留禁止其參與之權利而不作任何退款；

11. In case of any disputes, the Organiser reserves the right to make all decisions related to the event, including but not limited to the confirmation of entries, the arrangement of the event, and the amendment to the terms or conditions without prior notice; and 如有任何爭議，主辦單位保留對是次活動的最終決定權，包括但不限於取錄參加者及活動安排，以及保留增減、修改及刪除條款及細則之權利，而不作事先通知；及

12. All personal data provided by the participants is to be used solely for this event. The information will be provided to relevant staff for application processing, contact and other relevant purposes. In accordance to the Personal Data (Privacy) Ordinance, all personal data collected will be destroyed 6 months after the event. It is voluntary for participants to provide their personal data, but the registration cannot be processed without such personal data. 參加者所提供的個人資料只作本活動之用，有關資料只會供處理申請的人員參閱，作為審批、聯絡及相關的用途。所有資料均按個人資料(私隱)條例處理，並於活動期完結六個月後銷毀。參加者可依個人意願提供其個人資料。如參加者不願意提供相關資料，整個登記程序將不會完成，並視為未參加本活動。

All personal data provided by the participants is to be used solely for this event. The information will be provided to relevant staff for application processing, contact and other relevant purposes. In accordance to the Personal Data (Privacy) Ordinance, all personal data collected will be destroyed 6 months after the event. It is voluntary for participants to provide their personal data, but the registration cannot be processed without such personal data.

Chinese YMCA of Hong Kong undertakes to comply with the requirements of the Personal Data (Privacy) Ordinance to ensure that personal data kept are accurate and secure. Your personal information (including your name, email address, etc.) may be used by Chinese YMCA of Hong Kong for the purposes of sending you organisational updates, fundraising appeals, event invitations, surveys and other communication and marketing via telephone/post/electronic mails. If you do not wish to receive any communication from us, please email your Chinese and English full name, together with your membership number and telephone number to University and College YMCA Department at uniycy@ymca.org.hk.
